A few notes before filling out our form:
*Please talk to our intake coordinator if you do not live in Virginia and plan to see one of our clinicians via telehealth. We might have clinicians who can see you, but we will need to know this ahead of time.
*If you have TRICARE insurance, unfortunately, due to TRICARE's policies, if you submit for reimbursement, you and we are then bound by their contract. As a result, we have limited slots for TRICARE clients. If you want to talk more about this, let us know if you are a TRICARE subscriber and we can discuss this further.
